Club Application Form
The club application form is for student who would like to create their own club on campus. The club application requires (50) student signatures and an official full-time faculty member to be an advisor in order to complete this form. Please send this form to [email protected] for approval and next step options.

Club Continuation Form
A Club Continuation form services clubs who want to re-register their club for the next academic year. If your club has not filled out this form during the summer before the next academic year, your club will be put on hiatus until this form is completed.
