Club Application Form

The club application form is for student who would like to create their own club on campus. The club application requires (50) student signatures and an official full-time faculty member to be an advisor in order to complete this form. Please send this form to [email protected] for approval and next step options.

Club Continuation Form

A Club Continuation form services clubs who want to re-register their club for the next academic year. If your club has not filled out this form during the summer before the next academic year, your club will be put on hiatus until this form is completed. Please contact [email protected] with any questions.

The Clubs Rules and Guidelines serves as a form for clubs to refer to when they have any questions or concerns about the Clubs department on campus.
